Can we collaborate?

Supporting Independence works alongside other providers to help transitions succeed.


Planned transitions to remote support are most effective when providers collaborate, expectations are clear, and individuals are supported through change.


Supporting Independence’s role is to help stabilize routines, assess readiness, and guide transitions that support long-term independence.


This work complements the services of residential providers, home-based care providers, remote support teams, and other professionals within the system.


We enjoy hearing from other providers, and exploring opportunities to collaborate.
